Common forms

• Metal Screening Form: All participants need to be diligently screened for contraindications to MR imaging during recruitment. Review and sign the Metal Screening Form with each participant for each scan session.
• Late Cancellation Form: Scan Cancellation Form
• Pilot Application Form: Pilot form. Fill out this form if you would like to apply for scan hours toward a pilot project. Send the completed form to alenarto@g.ucla.edu and make sure to tell us the outcome of the pilot--if it helps you successfully secure funding, this information is crucial to keeping the pilot program going!
